Standard eye chart (letter size is exactly the same as Snellen eye chart) with white background and playful art of hot air balloons. 
Inspired by my time crewing for a hot air balloon team during college with the Dean of the College of Engineering at our state university. I also attended the Albuquerque Balloon Fiesta.
 
These creative eye charts are meant to ease the stress around measuring vision by being colorful, light-hearted and encouraging for the eyes to view. Designed by me.

Full-size vibrantly full-color premium matte poster vinyl white eye chart poster, printed on a $14,000 professional color printer for sign businesses.  (note that the quality of the eye chart pdf is not print quality for large format printing, it took me over eight hours to correct this as vision charts are not a normal font)

The ink is practically waterproof.
